REPORT UPON INVESTIGATION INTO THE EFFECTS OF COLD UPON THE BODY

Norman C. Lake conducted these investigations and found that a reinvestigation of "frost bite" was also necessary, for if the effects of "chilling" of the tissues is to produce a state of suspended animation, then it would be illogical to accept theories which assumed a direct effect of a degenerative and destructive nature.

The term "frost bite" is here used in its widest sense, and includes such conditions as "trench foot." He offers the results of his investigations as follows: (1- The temperature-60 C. must be considered critical in relationship to the effects of cold upon the tissues. (2) Degrees of cold below this temperature produce true frost bite and actual damage to tissues. (3) Degrees of cold above this temperature only produce effects secondarily by causing vasomotor paralysis. (4) Nerve blocking does not prevent either the effects of frost bite or chilling. (5) The use of vasoconstrictors delays the swelling in cases of true frost bite, while in cases of chilling the exudation may be entirely prevented. (6) Satisfactory treatment of trench foot must be directed towards preventing any appreciable rise in capillary pressure. (7) In the production of trench foot cold, must be considered the essential factor, other factors being either secondary or subsidary. (8) Treatment by the use of vasoconstrictors, either intravenously or otherwise, would appear logical and worthy of trial. (9) Treatment by nerve-blocking, either regional or central, is also suggested, although, in all probability, it cannot be expected to give good results unless used in conjunction with vasoconstrictors. (10) Prophylaxis consists in using any and every means of preventing congestion of the legs, and in avoiding, as far as possible,

variations in the temperature. (11) The question as to whether any surgical procedure, such as temporary occlusion of the arteries, would bring about the desired reduction in capillary pressure is being investigated.— Medical Record.

SPINAL SYPHILIS

The New York Medical Journal of June 16, 1917, quotes Lyon as stating that the treatment may be di vided into the management of the acute attack and the treatment of the intercurrent periods. During the acute attack treatment consists of absolute rest in bed, abstinence from oral feeding, continuous lavage, administration of antispasmodics, such as atropine and belladonna, to overcome spasm, supplemented by a mechanical maneuver directed to stimulate the spinal nerves, to overcome a vagotony which gives rise to acute dilatation of the stomach. This is especially emphasized. For relief of pain he advises a trial of adrenalin chloride intramuscularly with final recourse to morphine. During the intercurrent period the patient's health should be built up in the attempt to prevent or abort further attacks. Rest, hygiene, and hyperalimentative diet, tonics, and above all the intensive use of antisyphilitic therapy by intramuscular, intravenous, and intraspinal routes, should be used.-Therapeutic Gazette.
